Laser Vision Correction:

EyeMed and LCA-Vision have arranged to provide a discount to all EyeMed members through one of the largest laser networks available, the U.S. Laser Network. Members are entitled to 15% off the retail price or 5% off the promotional price of LASIK or PRK procedures, whichever is the greater discount.

Prescription Drugs:

Members have access to over 57,000 participating pharmacies nationwide through Caremark. Members have immediate access to average savings of 31% on generic drugs and an average savings of 13% on brand name prescriptions.

To access the discount, members simply present their membership identification card to a participating pharmacy. The pharmacist will then enter the information from the membership card into the online computer system and the member will be immediately eligible for substantial savings on prescription drugs.

Mail Order Pharmacy

Members can save more on their maintenance medications through Caremark's convenient and money saving mail order service through the website. To start using the Caremark.com mail order service feature, the member can download an order form from the website and send it along with the prescription and payment to Caremark.

When starting mail order the member should send the completed mail order form, the prescription and payment to Caremark. Orders typically arrive in 10 to 14 days.

Diabetic Management:

Hospital:

Galaxy Health Network Hospital (GHNH) program provides members with access to over 2,500 acute-care hospitals nationwide*. Members have the opportunity to save 10% to 30% or more. Members are able to use any in-network provider or facility as often as needed. To ensure members have a positive experience when using their medical savings card, network representatives are ready to help.
