Water Baptism

Many churches have requested a Water Baptismal Form.  A universal form helps a church to keep better records on their Baptisms, explain the meaning for Water Baptism to the applicants and parents, and provide a consent form on minors to be Baptized.  Minor consent forms are very important for Baptizing minors of parents who may not attend your church. Full article can be viewed and printed with Word or PDF formats at the links located at the bottom of this page.

As a Pastoral Care, Inc. goal, this application is written in a Word format so that you can easily modify or add specific information for your Baptismal service.  This form includes water immersion but can be easily modified to meet other forms of Water Baptism.
